In the evolving landscape of clear aligner therapy, clinical predictability and treatment efficiency remain top priorities for orthodontists. Two clinical case studies, courtesy of Ed Lin, DDS, MS, who practices in Green Bay, Wisconsin, highlight the versatility of the SureSmile Aligner system (Dentsply Sirona) in resolving common yet complex Class I malocclusions. These cases demonstrate how tailored digital planning can deliver highly precise esthetic and functional outcomes within remarkably efficient timelines.

The first case focuses on a 26-year-old female presenting with Class I spacing, rotation, and noticeable incisal wear. Diagnosed with slightly asymmetric maxillary and mandibular spacing alongside a prominent maxillary diastema, the treatment strategy prioritized arch symmetry and spacing closure. By moving the premolar-to-premolar segments (U4-4 and L4-4) and correcting crown rotations, Lin achieved complete closure of all spacing and leveled the incisal edges in 9.25 months, utilizing just 15 upper and 14 lower aligners.

The second case showcases the rapid correction of Class I crowding and a moderate deep bite in a 17-year-old female. Her presentation included mild maxillary crowding, moderate mandibular crowding, and uneven incisal edges. Lin’s treatment plan involved targeted movement of the U4-4 and L4-4 segments, interproximal reduction (IPR) distal to the lower premolars, and the leveling of the mandibular curve of Spee via the intrusion of the lower anterior teeth (L3-3). The treatment successfully resolved the crowding, opened the bite, and achieved arch symmetry in 4.25 months, using 8 upper and 11 lower aligners.
