Dr Giacomo Scuzzo details a case combining orthopedic and orthodontic phases to resolve a skeletal Class II malocclusion and deep bite in 9 months.
Treating complex skeletal discrepancies in growing patients often involves multi-phase approaches that can extend treatment timelines. A case study by Giacomo Scuzzo, DDS, demonstrates how an integrated aligner system can streamline this process. Using the Angel Aligner A6 Mandibular Advancement solution, Scuzzo combined orthopedic and orthodontic mechanics into a single, efficient phase to correct a Class II malocclusion in a pediatric patient.
The patient, a 10-year-old male, presented with a chief complaint of dental spacing and protrusive teeth. The clinical diagnosis confirmed a skeletal Class II profile due to mandibular deficiency, accompanied by an increased overjet, deep bite, and an upper diastema. Functional analysis revealed labial incompetence and suction of the lower lip. Dentally, he exhibited a Class II molar and canine relationship on the right side and Class I on the left side.
Scuzzo’s treatment plan utilized 25 Angel Aligner KiD 1 aligners, changed every 10 days. The protocol initiated simultaneous upper archform expansion and mandibular advancement from the start of treatment, eliminating a separate pre-advancement phase. The aligners incorporated occlusal blocks to guide the mandible forward while also leveling both arches and normalizing the overjet and overbite.
The treatment was completed in 9 months, successfully establishing a Class I occlusion and resolving the patient’s functional and esthetic concerns. Scuzzo noted the clinical advantage of performing two treatment phases within the same digital plan, which allowed for precise coordination of the upper arch development with the mandibular advancement. The patient reported a positive experience with no discomfort, highlighting the system’s potential to improve both clinical efficiency and patient compliance.
