FormDoctor, a new start-up located in San Francisco, has launched a new web application that allows patients to complete medical forms on their phone, tablet, or computer before their appointment.
Practices can use their existing intake forms to get started or use one of the company’s intake templates. FormDoctor will convert existing intake forms to FormDoctor at no cost. Once the form is set up, practices can text or email patients a link to the application. FormDoctor can also be embedded on the practice website. Additionally, patients can be given a tablet in the office to fill out the intake forms there.
Completed patient forms are stored securely on the company’s HIPAA-compliant servers. Practices can view submissions and print completed forms from their FormDoctor account.
