The American Dental Association (ADA) has introduced a Humanitarian Award, which recognizes dental members who have shown outstanding leadership and made contributions to patients and colleagues in the field of dentistry in the United States or abroad.

Any active, life, or retired member of the ADA may nominate a candidate by submitting a form to the Office of the Executive Director by October 15 of each year for consideration by the Board of Trustees. Nominations received after the October 15 deadline will be placed on file along with the required documentation for consideration the following year.

The following documentation must accompany each nomination:

• a fully completed nomination form, addressing the services of the nominee and the ongoing legacy that he or she will leave, taking into account specific actions employed, demonstrated leadership skills, knowledge, dedication, and persistence used to improve the oral health prospects of the specific communities, the population groups served, and the long-term impact of these endeavors;

• a resumÉ highlighting the nominee’s contributions;

• three letters of support from individuals other than the nominator;

• other essential documentation to substantiate and fully present the character and achievement of the nominee at the discretion of the nominator (testimonials, photographs, brochures, newsletters, or clippings); and

• the ADA Humanitarian Award Nomination Packet.

Nominations should be sent by mail, postmarked no later than October 15. They should be addressed to:

Nominations that are faxed or e-mailed will not be accepted. Incomplete nominations will not be considered. All nominations become the property of the ADA, which may use them for any purpose including program promotion, information dissemination, and professional education. Nominations will not be returned.
